Estimating Your Costs

Medicare provides a tool that allows members to look up the estimated costs for procedures. Procedures may be done in both ambulatory surgery centers (ASCs) and hospital outpatient departments. Estimates are calculated using either the procedure name, CPT, or HCPCS code.

FHCP Medicare follows the same fee schedule as Medicare, however your costs may vary by location. Prices shown are national averages, based on Medicare’s 2021 payments and copayments.

Your individual out-of-pocket costs may vary based on your deductibles, maximum out-of-pocket limits, co-insurance, or copayments which are not considered in the estimated costs provided by this tool.

Please be aware that your out-of-pocket expense may vary significantly by using in-network versus out-of-network benefits. You may also be responsible for charges by non-participating providers for non-emergency services.
